980 nm Diode Laser: A Good Choice for the Treatment of Pyogenic Granuloma

Abstract

Pyogenic granuloma is a benign non/neo plastic mococutanous lesion . It is a reactional response to constant minor trauma and can be related to hormonal changes. Several methods can be applied to remove PG like surgery electrocauterisation and laser treatment. We report our experience for the treatment of two cases with pyogenic granuloma with diode laser 980 nm. Patients reported no pain or swelling after the treatment. The wounds were completely healed without scarring, providing good aesthetic results. Diode laser 980 nm is a good modality for the management of pyogenic granuloma.

Frequency of Some of Oro-Facial Developmental Disturbances & Abnormalities among the Newborns in Kerman, Iran: A Retrospective Study

 

Abstract

Aim: Oro-facial and cranial malformations are important health problems in children. These abnormalities are major cause of infant mortality and childhood complications. The aim of this study was to evaluate oral, facial and cranial abnormalities in newborns in Afzalipour Hospital in Kerman, Iran.

Materials & Methods: All cases of newborns were reviewed, and oral, facial, and cranial abnormalities were recorded in a checklist. The information in the checklist consists of two parts:

a) Information about the mother including age, maternal health status before delivery and type of delivery.

b) Information about the newborn (sex, weight, type of abnormality and health status of the newborn at birth). The results of the case review be analyzed by SPSS21 software.

Results: In this study, 6225 cases were reviewed, which includes 3015 normal deliveries (48.4%) and 3210 cesarean deliveries (51.6%). The most important results:

a) Head, cranium, and neck abnormalities & deformities: 3 newborns (0.048%)

b) Cleft lip and palate: 3 newborns (0.048%). Natal teeth: 2 newborns (0.032%). Facial asymmetry: 3 newborns (0.048%)

Conclusion: The prevalence of cesarean delivery in Kerman is high. Prevalence of natal teeth and cleft lip and/or palate is almost similar to many of countries

Has Covid-19 Global Pandemic Impacted Antibiotic Use Among High Caries Risk Children? Evidence from Preventive Oral Health Care Setting in Sri Lanka

 

Abstract

Background: Covid-19 global pandemic ravaged health and wealth of people in developed and developing countries crippling health care systems and economies across the globe. Despite countries resuming normalcy powered by mass scale immunization drives after battling sinister waves of Covid-19 epidemics, its pervasive impact is yet to be fully explored. Early childhood caries (ECC) ranks top among the leading chronic childhood diseases especially among high risk toddlers and children in developing countries and disadvantaged minority groups in developed countries. Covid-19 induced interruptions into preventive oral health care services compounded by lock-down related life-style changes fostering unhealthy dietary patterns were common scenarios in those troubled times. Against this backdrop, present study attempts to explore and expound the impact of Covid-19 milieu on antibiotic use among high caries risk toddlers and children attended a public preventive oral health care unit.

Method: The study design was retrospective, hospital based, and the study setting was the Preventive Oral Health Unit of the National Dental Hospital (Teaching) a premier, multispecialty, tertiary care public dental hospital in Sri Lanka. The patient statistics data base for the year 2019 was used as the pre-Covid-19 baseline, compared with the years of 2020 and 2021 affected by Covid-19 global pandemic. The proportionate episodes of child-patient visits needed antibiotics for dento-alveolar infections to the total number of child-patient visits from 1st January to 31st December each year and during peak Covid-19 periods in 2020 and 2021 were compared. Data were entered and analysed using SPSS-21 Statistical Software Package. Frequency distributions and descriptive statistics were used for data presentation. The means were compared by independent sample T-Test, Mann-Whitney U test and one-way ANOVA test of statistical significance after assessing the distribution of data by Kolmogorov-Smirnov and Shapiro- Wilk tests of normality.

Results: In overall, there was a proportionate increase in presentation of symptomatic dento-alveolar infections to total visits during the peak periods of Covid-19 waves in 2020 & 2021 compared to respective months of pre-Covid 19 era (year 2019). The mean percentage proportion of prescribing antibiotics to total number of visits was 18.07 for the year 2019 whilst it was 31.25 and 32.73 for the years 2020 and 2021 respectively. Those indicated increased use of antibiotics in Covid-19 mileu compared to the baseline year. However, those differences did not reach statistical significance (p=0.078) in 3-year comparison. Nevertheless, there was a statistically significant increase (p=0.006) in percentage proportion of prescribing antibiotics to overall visits in 2021 compared to 2019 however, the increase of prescribing antibiotics in 2020 compared to 2019 did not reach statistical significance (p=0.319). Furthermore, comparisons of antibiotic use in months of peak Covid-19 epidemic periods in 2020 and 2021, with corresponding periods of pre-Covid 19 era showed statistically significant increases (p=0.007 and p=0.0001).

Conclusions: As evident from the findings, Covid-19 milieu in its peak periods, significantly impacted the use of antibiotics for dento-alveolar infections of high risk toddlers and children favouring an over- use, which could be attributed to interruptions to proactive preventive oral health care service utilization. As overuse of antibiotics could aggravate the existing global public health threat of antimicrobial resistance (AMR), this becomes a cause of concern. However, further studies are warranted to confirm the evidence generated from this study

Access to Oral Health Services in School Children in Camaçari- Bahia- Brazil

 

Abstract

Objective: To describe access to oral health services in public schools in the age group from 2 to 19 years old, in the city of Camaçari, Bahia.
Methodology: This is a cross-sectional study involving the participation of 1321 parents of schoolchildren aged between 2 and 19 years old enrolled in public schools in Camaçari, a municipality in the interior of the state of Bahia with approximately 300,000 inhabitants. The collection of data identified in the years 2017 to 2019 and consists of a previous survey with oral parents in the population’s access to health services, as well as oral health habits. Participation was conditioned to the signing of an informed consent form by the parents/guardians.
Results: 19% of the study population were female. Most students (67.24%) are up to 10 years old, 32.76% were between 11 and 19 years old. Most 51.31% reported never having gone to the dentist. Almost all did not have dental plans (93.99%). There was significance in the chi-square test with the variables: sex (p-value= 0.00); having a dental plan (p-value= 0.00); and use of dental floss (p-value = 0.002).
Conclusion: The lack of access to the health service demonstrates a need to prioritize the age group in health actions in the PNSB and PSE. A socio-demographic and socio-environmental heterogeneity of the target audience should be considered when being in education for the same health, intervention measures and becoming effective and comprehensive mainly by enabling access to oral health.

Treating Inflammatory Root Resorption Following Endodontic Regeneration – Case Repor

 

Abstract

Aim: To describe complication management following regenerative endodontics.
Introduction: Regenerative endodontic procedures (REPs) are aimed to treat apical periodontitis and promote root maturation of immature necrotic teeth. Recently it was shown that REP may arrest inflammatory external root resorption (IERR) in replanted avulsed teeth. The purpose of this study is to describe a complication of REP in which IERR had been developed after REP treatment. Root Canal Treatment (RCT) using bio ceramic materials helped to arrest the IERR.
Methods: An eight-and-a-half-year-old girl was referred for treatment after avulsion of tooth #9 which has been splinted in a hospital for 2 weeks. After thorough examination tooth #9 was diagnosed as having necrotic pulp with symptomatic apical periodontitis. The tooth was treated by REP using plasma rich fibrin (PRF) and restored. Eight months later, IERR has begun. RCT using trichloroacetic acid (TCA) and mineral trioxide aggregate (MTA) plug was performed. The resorption area was treated with bio ceramic putty material.
Results: 35 months after the treatment the tooth is functioning without signs and symptoms
Conclusions: The present case demonstrate a method in which a conservative RCT with some modification may arrest IERR using TCA. IERR may develop long time following REP treatment. A thorough long follow-up is needed after REP. An early detection of IERR may overcome this complication.
Key learning points: Although very rare, complication after REP such as IERR may occur, an early diagnosis of such complication may be resolved by modified conservative RCT.

Surgical Management of Massive Dentigerous Cyst in Mixed Dention- A Case Report

 

Abstract

A dentigerous cyst is an odontogenic cyst thought to be of developmental origin associated with the crown of an unerupted tooth. Such cysts remain initially completely asymptomatic unless when infected. It is detected clinically when becomes large and associated with swelling of the face. The purpose of this case report was to describe the diagnosis and management of a dentigerous cyst in an 8-year-old boy. The chosen treatment was cyst enucleation and tooth extraction.

Management of a Dentigerous Cyst in a 6-Year-Old Child – A Case Report

 

Abstract

Dentigerous cysts are epithelial in origin and most common odontogenic cysts. They are usually asymptomatic and hence diagnosed on radiological examination. The standard treatment for these cysts is enucleation and extraction of the affected teeth. This is a case report of a 6-year-old female patient with dentigerous cyst associated with a primary molar. The cyst was enucleated and unerupted premolars were removed from the lower left region. The patient was given a fixed functional band and loop postsurgical treatment. No recurrence was observed after 6months follow up.

Pinch of Prevention in Pounds of Troubles: Utilization of Preventive Oral Health Care Services by High-Risk Children and Other Target Groups Amidst Covid- 19 Milieu

 

Abstract

Introduction: COVID-19 global pandemic caused suspension of routine and preventive dental treatment whilst prioritizing emergency care. Preventive oral health care services contribute to oral health improvements among high-risk children and pregnant women. We, therefore, aim to investigate the impact of COVID-19 on utilization of preventive oral health care services targeted for high-risk groups provided by a premier tertiary care public dental hospital in Sri Lanka.
Methods: A retrospective cross-sectional study on performance statistics of the Preventive Oral Health Unit of National Dental Hospital (Teaching) Sri Lanka was conducted for the periods from 1st January to 30th November 2020, pertaining to pre-COVID- 19 period (baseline), 1st Wave of COVID-19, transition period and on-going second wave. Comparisons were done on mean numbers of selected preventive treatment episodes performed and risk groups attended using independent sample t-test, one-way ANOVA and Kruskal-Walli’s test. Relative contributions of emergency visits for total visits were assessed for the given period. Data were entered and analysed by using SPSS-21 statistical package.
Results: Significant declines was evident in preventive dental care episodes provided for high-risk toddlers and pregnant mothers during the first and on-going second waves of COVID-19 compared to pre-COVID- 19 and transition periods(p<0.05). In contrast, substantial increases detected in relative contribution of emergency visits for total number of visits among children for symptomatic dento-alveloar infections during two waves of COVID-19 compared to pre-COVID- 19 and transition periods.
Conclusions: COVID-19 has negatively impacted on preventive oral health care service utilization by high-risk groups, whilst raising the proportion of emergency visits due to aggravated dental diseases. Providing preventive care becomes essential to reduce the burden of aggravated late sequel of dental caries among high risk children. Novel models of preventive oral health care services for high-risk low socio-economic groups could be a way-forward in COVID-19 milieu.

Can Genetics Play A Role in Children’s Tooth Decay?

 

Abstract

Many people still get decay despite regular care of their teeth. Studies over the years have shown genetics has been very influential in the health of teeth. Some people think that genetics only affects the materials of the teeth, while genetics also affects the amount of sugar consumed. Studies have shown that many people are genetically more likely to consume sugars, and their metabolism is adjusted to consume these sugars. Microorganisms are another factor that can cause tooth decay. Our immune system determines what microorganisms are naturally present in our mouths. The immune system is based on genetics, so genetics can affect tooth decay through the immune system and the presence of bacteria. Although the role of genetic factors in the development of dental caries is well established, the role of environmental factors should not be underestimated. That’s why in this article we will talk about the role of both genetics and environmental factors in tooth decay.

Analysis of Marginal Bone Loss and Implant Success Rate After Implant Implantation

 

Abstract

Purpose: Patients with prosthesis-equipped post-implantation prosthesis performance were continuously observed, revealing the association of marginal bone loss with gender and age implant success and survival rates during a 1year study of clinical prosthesis placement; with results used as a reference for implant procedures and prognosis.

Methods: This study was conducted on 31 patients from January to February 2021 at Y Dental Hospital in Gwangju, with 20 men and 11 women equipped with prosthetics after implant procedure a year earlier. The subjects of the study were carried out with the G-power program, and 31 subjects were extracted. The distance from the shoulder of the implant to the attachment point of the dichotomous bone was measured at the center of the area of atrophy, checking both from the mesial region to the distal region using a radiation ruler taking photos with endocardial radiation as well as panoramic images. The subjects of the implant study were analyzed by age, gender, cause of release, duration of implant, implant length, implant width, implant assembly period, osteoplasty type, shield presence, match with corresponding teeth, implant survival symptom, implant success rate symptoms, and 1mm loss within a year. This study was conducted after approval.

Results: As a result of testing the contribution and statistical significance of individual independent variables to dependent variables, at a significance level of 0.05, the independent variable loses 1 mm at a ratio of (t=5.473, p=.00) in 5.473, p=.000). This loss significantly affects implant success rates.

Conclusions: Recognizing the importance of bone loss management within a year after implant procedure, dental staff and patients can achieve higher success and survival rates after implant procedure if they can be thoroughly managed within a year.

Periodontal Disease in Mother and Consequences for both Mother and Baby

 

Abstract

Periodontal disease is a type of bacterial infectious disease that causes bone destruction and supportive fibers of periodontal tissues. It seems the prevalence of this disease is more common in adults and would increase during pregnancy. Researchers believe hormonal changes are the main factors of developing this disease. Inflammation or infection of the gums increases the risk of low birth weight and preterm birth. If the problem is left untreated, mothers who would experience severe oral infections and gingival bleeding during the pregnancy would be at a greater risk for preterm birth.

A Study on the Coronavirus-19 Response of Implant Patients and Dental Workers

 

Abstract

Objective: The Dental clinic studies Infection control and Differences in Hygiene Management in Dental Manager to provide data based on safe Dental clinic by enhancing the Prevention of Infectious Diseases in Immigration and Dental Manager.

Methods: The study was conducted in January 2021 by 30 implant patients and 30 permanent workers who were receiving post-implant care in the G city area. The Implant patient’s coronavirus–19 prevention questionnaire includes age, gender, scaling after implant implantation, 1mm Loss of Limbic bone, wearing a mask, body temperature measurement, and disinfectant. Fourteen questions for Dental workers under management include scaling after implantation, Dental cartilage Loss 1MM, Dental hygiene age, mask repetition, indoor ventilation, disinfectant hand washing, paper towel use, medical gloves, gloves disposal, kf94 mask, mask disposal, eye protection, and disposal.

Results: In a simple regression analysis, the independent variable that significantly affects the wearing of kf94 masks at a significant Level of .05 is the age of Dental hygiene (t=-2.259, p=).It was significantly shown as 033). However, wearing a mask repeatedly (t=-).626, p=.537), Ventilation(t=-1.697, p=.102.), Wash hands with disinfectant (t=).264, p=.There is no significant difference in 794.

Conclusion: The new study is very significant, and we conducted a new perspective to check closely with Dental managers to see if there is thorough prevention of infectious diseases in order to prevent cross-infection from implant patients to Dental managers in COVID-19 situations

Surgical Management of Impacted Inverted Mesiodens With cyst Excision

 

Abstract

Impacted tooth is most frequently diagnosed during the routine dental check-up, commonly associated teeth are third molars, but impacted inverted mesiodens are considered to be one of the infrequent findings in dentistry. Here with, a rare case is reported which is suspected of inverted mesiodens describing its surgical intervention along with cyst removal which was detected in radiographic examination and also showing importance of radiograph in diagnosis of infrequent cases in dentistry.

Periodontal Diseases in Children

 

Abstract

Periodontal disease in children is a condition in which there are destruction and resorption of the supporting bone of the teeth, which has three types: local, diffuse, and pre-pubertal. The local type of severe bone resorption is symmetrically and bilaterally in the deciduous dental arch, which is not accompanied by inflammation of the gingival tissue and plaque. The causative agents are hereditary blood disorders along with bacterial agents. In the diffuse type, which occurs mostly in puberty and progressively, symptoms such as gingivitis, plaque, and calculus are evident. Treatment for these two conditions is surgery with the use of Tetracycline and sometimes Metronidazole. Local bone resorption is mainly the cause of systemic diseases such as blood disorders, leukemia, diabetes, and inherited Syndromic diseases. Gingival recession can be caused by chronic inflammation due to poor hygiene, the presence of orthodontic stimuli and movements, and oral habits. Sometimes gingival resorption is caused by self-harm for psychological reasons that the child damages the gingival tissues. The third category is the pre-pubertal type, which occurs at the age of 4 during or after the growth of mammary glands, which sometimes occurs early due to the severity of tooth decay. Treatment is early diagnosis, curettage, massage, and health education. Sometimes broad-spectrum antibiotics are necessary.

Experiences of Mothers of Affected Children on Family Impact of Amelogenesis Imperfecta: Findings of A Qualitative Explorative Study From Sri Lanka

 

Abstract

Background: Amelogenesis Imperfecta (AI) denotes a heterogenous group of conditions characterized by inherited enamel defects among children which affect the structure and clinical appearance of the enamel of teeth of primary and permanent dentitions. Hence, it is well known that AI negatively impact on families and parents especially mothers and fathers as primary caregivers of affected children. However, little is known on the context in developing countries in this regard. Objective: To explore the experiences of mothers in Sri Lanka: a developing middle-income country on the family impact of having a child affected by Amelogenesis Imperfecta (AI).

Study design: A qualitative, explorative study was conducted among mothers of children having confirmed diagnoses of AI attended the Pediatric Dentistry Department, Faculty of Dental Sciences; University of Peradeniya. A series of semi-structured interviews were conducted among mothers of affected children prior to commencing treatment. The Family Impact Scale (FIS) of Locker 2002, was used for the interviewer guide. Thematic analysis was conducted to analyze the transcribed interviews.

Results: Participating mothers expressed their deep concern about having a child with AI in terms of disturbed parental/family activities, negative parental emotions and family conflict. Furthermore, mothers were much worried about quality present and future of life of their children.

Conclusions: Present findings supported the need for emotional support for mothers receiving treatment for AI. Moreover, suitability of the 4-dimensional- Family Impact Scale as a theoretical framework for family impact assessment of having children with AI in a developing country context was indicated by the findings. Explicit theoretical framework-based family impact assessment might enhance child and family- centered treatment outcomes in longstanding management of children with inherited developmental defects of teeth and craniofacial complex such as AI. Nevertheless, further research warranted to confirm the present findings.

Pallor and Whitening of the Gums in Children

 

Abstract

Whitening of the gums often indicates that there is a problem with the health of the mouth. Many conditions can lead to gingival whitening, from Aphthous ulcers to long-term inflammatory disease. This condition may be accompanied by some signs and symptoms. In some rare cases, whitening of the gums can be a sign of oral cancer; hence, there should be a concern of the parents to see a pediatric dentist for a correct diagnosis.

Self-Inflicted Oral Mucosal Injuries in Pediatric Patients: A Case Series

 

Abstract

Self-Injurious Behaviour (SIB) is a deliberate harm that may lead to factitial injuries in various parts of the body including the oral mucosa. It may present with a confusing clinical appearance leading to misdiagnosis as a variety of oral diseases [1]. A case series is presented here outlining 2 cases of self-induced oral mucosal injury, which presented initially with extensive oral ulcers. These cases were investigated to rule out systemic or local causes, in order to establish an accurate diagnosis. A multidisciplinary approach to treating these conditions, which may be associated with several medical conditions, is necessary, necessitating the engagement of patients, parents and medical and dental teams [2].
